

These forms confirm that you had health insurance coverage and are issued by different organizations depending on the source of your coverage:
Form 1095-A: Issued by the Health Insurance Marketplace (also known as the Exchange) to those who purchased insurance through it. This form is mandatory for filing your tax return, as it is used to calculate the Premium Tax Credit.
Form 1095-B: Provided by insurance companies or government programs (such as Medicaid, Medicare) to individuals insured outside the Marketplace.
Form 1095-C: Issued by employers to their employees, indicating the health coverage that was offered to them.
